To submit your federal ID application, you must choose an acceptance facility where you will submit your completed application sometime after the clinic. Acceptance facilities include post offices, clerks of court, public libraries, and other local government offices which accept passport applications on behalf of the U.S. Department of State. The facility name you list on this form, MUST be the facility where you submit your federal ID application. Your facility location cannot be changed after submission of this form.

If your application is accepted, you must complete a financial need self-certification statement asserting that you make less than 200% of the Department of Health and Human Services poverty guidelines. For example, a household of 1 person must have an income of less than or equal to $29,160 per year; a household of 2 people must have an income of less than or equal to $39,440 per year; etc.
