Appointments are limited and will be filled on a first come, first served basis. To secure an appointment, you must:

• Complete this registration form, then

• Book your appointment time using the link in the confirmation webpage or email you receive.

NOTE: If you do not complete the second step to book your appointment time, your appointment at the clinic will not be reserved. In the event that all appointments are filled, we will email you to let you know.

After you complete your application forms with us, you will still need to take your documents to an acceptance facility to submit your passport application. At the clinic, we can help you locate the most convenient acceptance facility.
