1243 - 1313 AR-15-6 Investigation 4/8/2004 Summarizes an investigation into allegations of detainee abuse by Regional Team Chief who reported that a detainee was prevented from using the toilet, and that other detainees were held under temperatures of 52 degrees and denied food. (1255). The investigation concludes there was no mistreatment of the detainee (1243). The investigating officer strongly criticizes the Team Chief for reporting the abuse directly to BG Hood instead of reporting up the appropriate chain of command. (1255-1258). The file contains recommendations that the officer reporting abuse be immediately relieved of his duties as a regional team chief and receive a reprimand for his failure to follow the chain of command (1260).
1314-1315 Memorandum for Commander, United States Southern Command from General Geoffrey D. Miller re: Commander's Inquiry, Allegation of Inhumane Treatment of REDACTED. 5/3/2003 Memorandum from General Miller, dated May 3, 2003, noting that he had on May 2, 2003 directed the Director of the Joint Intelligence Group, sto "immediately cease the use of the Fear Up Harsh" interrogation technique, that MPs should no longer participate in interrogations, and only DOD personnel may review and approve interrogation plans. (1314). The memo states that "I found that the action involved in this commander's inquiry was a single incident. A thorough review of our current procedures has not uncovered a systematic problem." (1314).
1316-1317 Memorandum for John Antonius, Director, Joint Intelligence Grou, from General Geoffrey D. Miller re: Commander's Inquiry, Allegation of Inhumane Treatment 5/2/2003 Memorandum from General Miller, dated May 2, 2003, for Joint Intelligence Group, stating that interrogators should "immediately cease the use of the Fear Up Harsh" interrogation technique, that MPs should no longer participate in interrogations, and only DOD personnel may review and approve interrogation plans. (1316).
1318 - 1322 Memorandum for Major General Geoffrey D. Miller, Commander, Joint Task Force Guantanamo Bay, Cuba from Staff Judge Advocate [name redacted] re: Results of Commander's Inquiry re: Allegation of Inhumane Treatment of [redacted]. 4/30/2003 Summarizes interviews relating to April 22, 2003 incident of detainee abuse at Guantanamo Bay. According to the interrogator who witnessed the abuse, "they [MPs] pushed in the back of the detainee's knees with their knees, taking the detainee to his knees. Then holding the detainee by his upper arms they slammed his upper body to the floor." (1318) "This procedure was repeated 25-30 times" and "the floor was shaking with such force that [redacted] as well as the next booth over were shaking [redacted] stated that as the detainee was forced to the floor, he turned his head so the side of his face was hitting the floor. He further stated that the force being used on the detainee caused REDACTED great concern for the detainee's safety." (1318) A female witness "further stated that she witnessed [redacted] and laughing while this activity was taking place." According to one of interrogators who participated in the interview, "the technique was appropriate, approved, applied properly, and was common practice." (1319). The MPs denied that the detainee's head was allowed to hit the floor (1320). The use of MPs in interrogation was apparently approved by a "previous JIG commander." (1320) "Military police stated that they have [standard operating procedures] for Camp Delta but no guidelines for the interrogation areas. They stated that they do not know what the limitations [are] and whether they should aprticipate in any aspect of the interrogation process. (1321).

1327-1328 Memorandum for Record. Subject: [redacted] 4/15/2003 Includes information about detainee: intelligence summary, psychiartic and medical history, medications, impressions of detainee, and a recommendation for detainee to be interrogated.
1329-1332 Memoranda related to abuse Incident on 22 Apr 03. Parties redacted. 4/26/2003 Summarizes an inquiry into an interrogator's allegation that military police physically abused a detainee at the direction of another interrogator. "As an experienced interrogator, who has been trained and served in the USA Army, I have never seen in FM 34-52 any section describing or prescribing what he had done to the detainee." (1330).
1333-1334 Memorandum for Recrd re: Possible Inappropriate Activites 4/26/2003 Lists several incidents that took place in March-April 2003, including the abuse incident on April 22, 2003. The complainant of that incident appears to be an ACS employee. (1333). Of the other incidents mentioned, one involved "the proverbial strip club lap dance." (1333). Others invovled forced PT [physical training], including ups and downs.
1335 - 1346 Statements of witnesses to the April 22, 2003 detainee abuse incident   The interrogator who conducted the interview of the detainee states that he used "Fear Up Harsh" and that "regulations as well as the detainee safety was taken into account during the interrogation session at all times." (1335).
1347-1350 Marrative Medical Summary of Detainee 4/26/2003 Summarizes medical check-up of detainee. Detainee reports that "Reservation has 'dragged me across the floor' resulting in scrapes in his feet, jerked on the chains which have caused marks on his wrists, lifted him up then 'slammed' him down on his knees, made his mouth such that he was spitting up blood, made a tooth loose, bruised several areas on his upper arms and torso, and created pain on his L lower ribs." Medical examiner concluded that "Stated source of injury for the various areas is consistent with the injuries noted today."
