Article 15-6 Investigation of CJSOTF-AP Combined Joint Special Operations Task Force - Arabian Peninsula] and 5 th SF [Special Forces] Group Detention Operation (Formica Report) Iraq Report by BG Richard Formica of detainee treatment by Special Operations units in Iraq. Finds that interrogators used unauthorized techniques such as sleep deprivation, dietary manipulation and stress positions. Detainees were kept in four-foot by four-foot cells and fed only bread and water for up to 17 days. Report concluded that "policy guidance" was to blame and recommended some changes including better training. Dated November 8, 2004.

Court Martial Records of Alan R. Taylor Iraq Failed to prevent the locking of Iraqi detainees inside an abandoned tank. He himself locked them in the tank also. Failed to stop Corporal Scott A Burton from spraying a fire extinguisher into a prisoner's face. He himself sprayed a fire extinguisher into a prisoner's face. Failed to prevent Corporal Scott A Burton from lining-up detainees in front of foxholes and then firing his gun next to a detainee's head. He himself fired a round next to a detainee's head. PLEA: Guilty to preventing Corporal Burton from committing the mentioned offences. Not guilty to committing the offences himself. RESULT: Guilty of all charges. Reduction to pay grade E-3 and hard labour without confinement for 30 days.
Court Martial Records of Matthew K. Travis Iraq Along with Trefny, Gabbey and Sting, conspired, witnessed and encouraged electrocution of a detainee by PFC Andrew J Sting, then lied about being present in an official statement. PLEA: Guilty to “failing to treat detainees with dignity and respect” or “intent to deceive”. RESULT: Reduction to pay grade E-1, confinement for 15 months and discharge from the navy.
Court Martial Records of Jeremiah J. Trefny Iraq Along with Trefny, Gabbey and Sting, conspired, witnessed and encouraged electrocution of a detainee by PFC Andrew J Sting. He then disobeyed an order not to discuss information about the subsequent investigation and knowingly lied in an official statement when he said “Sergeant Travis was not present” at the time of the incident. PLEA: Guilty to all charges, except charge VI of aggravated assault. RESULT: Confinement for 8 months. Forfeit of $795 pay per month for 8 months. Reduction to pay grade E-1 and separation from the Marine Corps.
